Referral Process
Our agency provides both group home and independent housing for persons with severe and persistent psychiatric illnesses. Our group homes range from transitional to long-term and provide services to both MICA and non-MICA consumers.

Co-Occuring group homes provide 24-hour/7 day per week staff supervision (Level "A+").
Inclusionary Criteria:
- Individuals must have a serious and persistent mental illness diagnosed on Axis I of the Diagnostic Statistical Manual of Mental Disorders, 4th ed. (DSM-IV), such as Schizophrenia, Schizoaffective Disorder, Bipolar Disorder or Major Depression. 1b. A co-existing, diagnosed substance abuse/addiction disorder.
- Persons must be an adult18 years of age or older.
- Must be an Essex County resident.
- Is capable of developing skills enabling him/her to live in a less restrictive, more independent environment. (e.g. medication management, transportation, and other activities of daily living).
- Willing to be engaged in daily, goal-directed activity (e.g. partial care, vocational training, employment, and school).
- The individual must demonstrate sufficient psychiatric stability such that s/he does not require inpatient services (i.e. not a danger to self, others, or property).
- Assessed to have a Global Level of Functioning of 4 or above (does not require institutional care or others to be responsible for most self-care functions).
- Free of serious medical conditions that would require home-specialized staff and/or home nursing care.
- Free of communicable diseases as certified by a medical doctor.
- Individuals must be substance-free for three months before referral and be willing to engage in a substance abuse/addiction treatment program.
- For individuals who have not achieved or maintained sobriety at time of referral, Project Live may request that he complete an inpatient detoxification or MICA rehabilitation program before admission into the program.
- Is able to comprehend and agree to enter into a residential program service agreement that identifies the contracting parties’ rights and responsibilities.
- For Ridge Street House, s/he must fall within low-income guidelines as defined by HUD (department of Housing and Urban Development).
- For MICA/450 Group Home (Roseville), at the time of referral the individual is an inpatient at a state psychiatric hospital. and be hospitalized for at least 4 months at that facility.
Exclusionary Criteria:
- Persons with a primary diagnosis of Personality Disorder, Organic Brain Syndrome, Substance Abuse (without a concurrent primary diagnosis as indicated in item 1 of the inclusionary criteria).
- Individuals with a history of arson (fire setting).
- For HUD funded properties (Lincoln Ave and Ridge St Group Homes and some SHP homes, persons with convicted felony charges
- Persons with symptoms and/or behavior that presents a danger to self, others, or property.
- Persons with a history of homicide, attempted homicide or patterns of violent behavior, including sexual assault/molestation will be assessed as to the clinical appropriateness of the referral.
- Persons with medical conditions requiring skilled nursing care.
- Individuals with a history of non-compliance with treatment.

Application Procedure:
- Referrals may be made by families, providers of mental health services and other social services agencies.
- Applicant will be asked to sign release of information during first interview, to enable Project Live to obtain background information.
- Once the completed application is received it will be thoroughly reviewed. You will be contacted in writing as to its disposition.
